FAIR Health Privacy Policy

Your privacy and the privacy of all users of the FAIR Health website are very important to us. We have adopted the following Privacy Policy for this Site (together with any successor site(s), the "Site"). The Site is owned and operated by FAIR Health, Inc. (“FAIR Health,” "we," "us," "our"). By using the Site you, as an individual and on behalf of your company or institution (“you,” “your”) agree to the collection and use of your personal information by us in accordance with the terms of this Privacy Policy.
Changes to the Privacy Policy
We reserve the right to change the terms of this Privacy Policy at any time. Any changes will be posted on this page; therefore, we encourage you to check this page regularly. You can determine when this Policy was last revised by referring to the "LAST UPDATED" legend at the top of this page. Your continued use of the Site following any changes to this Privacy Policy will constitute your acceptance of such changes.
Information We Collect
You can visit and browse the Site without revealing your own personally identifiable information. You also may choose to disclose personally identifiable information about yourself such as your name, postal address, telephone number and your e-mail address and we will collect this information when you actively give it to us. For example, if you send us an e-mail, we collect the information in that communication (such as your e-mail address and name), and such information may be personally identifiable.   
We may collect "non-personally identifiable information" such as your website browsing patterns, browser variables (for example, media access control (MAC) address, computer type (Windows or Macintosh), screen resolution, operating system version, Internet browser and Internet browser version), IP address or demographic information. We and our service providers may collect such non-personally identifiable information from you by "passive" means, often through the use of cookies, "pixel tags," "web beacons," "clear GIFs" or similar means. In addition, sometimes the Site may set a "session cookie" to help us administer the Site. A session cookie expires when you close your browser and does not retain any information about you after it expires. We and our service providers also may use so-called "pixel tags," "web beacons," "clear GIFs" or similar means in connection with Site pages and HTML-formatted e-mail messages to track the actions of Site users and e-mail recipients on the Site and to compile aggregate statistics about Site usage. We also may collect demographic information that you provide to us, such as your zip code or professional background. We use your demographic information to tailor the Site to users’ needs and preferences and for our internal purposes. Additionally, we may aggregate personally identifiable information so that the aggregated information does not personally identify you or any other user of the Site, such as by using personally identifiable information to calculate the percentage of our users who have a particular telephone area code. We consider aggregated information, both information aggregated by us and aggregated information obtained from third parties, to be non-personally identifiable information. 
This Site is not intended for use by children. We do not solicit or knowingly collect personally identifiable information from children under the age of 13.  If we receive actual knowledge that we have received such personal information, we will delete the information from our database as soon as possible.
How We Use and Disclose Information Collected on the Site
We will use your personally identifiable information for customer service and to customize our users’ experience on the Site. Your personal information is used by us to respond to your questions, inquiries, comments and instructions and to fulfill your requests. In addition, we may use your personally identifiable information to provide services and support to our users, improve our Site, process transactions, send related communications, as well as for other internal business purposes, such as data analysis, audits, developing new products, enhancing our Site, improving our services, identifying usage trends and determining the effectiveness of our promotional campaigns.
In addition, unless you tell us not to, we may use your personally identifiable information to contact you with information we think might interest you. If you do not want to receive this information, indicate that we may not do so at the point when you give us your personally identifiable information. If you change your mind later, e-mail us at info@fairhealth.org. Promotional e-mail messages provide you with an opportunity to opt-out of receiving this information. Please note that changes to your preferences may not be effective until ten (10) days after receipt by us.
We do not disclose any personally identifiable information that we collect from the Site to third parties for their marketing purposes without your prior consent. 
We may disclose personally identifiable information to third-party service companies to help us operate our business and the Site or to administer activities on our behalf and may share your information with these companies for those limited purposes.
We also may use and disclose personally identifiable information about you in connection with legal or regulatory requirements, such as in response to any authorized subpoena, governmental request or investigation, as required by law, to enforce our terms and conditions and to protect the interests and safety of FAIR Health or other users of the Site.  
We use non-personally identifiable information for internal purposes and to help us interact with visitors to the Site. We also may share non-personally identifiable information in aggregate form with third parties to illustrate trends and patterns. Because non-personally identifiable information does not identify you, we reserve the right to use and disclose it for any purpose. If FAIR Health or any of its assets are sold or if it is merged with another company or if it declares bankruptcy, some or all of the information collected from your information may be transferred to a third party as a result of such event. 
The Site may contain links to other websites. We are not responsible for the information collection practices or the content of the websites to which our Site is linked that are not owned or operated by us. We encourage you to read the privacy statements provided by those other websites before you provide personally identifiable information to them.   
Protecting your personally identifiable information is important to us. We seek to have security measures in place to protect the security of your personally identifiable information. Unfortunately, no data transmission over the Internet or data storage system can be guaranteed to be 100% secure. As a result, while we are committed to protecting your personally identifiable information, we cannot ensure or warrant the security of any information you transmit to us through or in connection with the Site or of information stored by us. You acknowledge and agree that any information you transmit through the Site or upload for storage in connection with the Site is so transmitted or stored at your own risk.  

For Research Portal Users Only
You can visit the FAIR Health Research Support Program website (hereinafter the “Site”) without revealing your own personally identifiable information. However, if you want to browse certain restricted portions of that Site, you will be required to provide your contact information. If you want to license any data for research purposes using that Site, you must complete the application and disclose personally identifiable information about yourself, the Principal Investigator of the research project and the researchers (collectively, “you”), including name, the name of the institution licensing the data (if applicable), postal address, telephone number(s), location where the data products will be accessed and used and e-mail address(es). When you send us an e-mail, we collect the information in that communication (such as your e-mail address and name), and such information may be personally identifiable.
